Volunteer Requirement
Fleetwood Fastpitch Association (FFA) is a volunteer association. Registration fees are calculated based on a parent or guardian performing four hours of volunteer time per child. If volunteer time is not performed, FFA will cash your volunteer cheque to recoup the full registration fee.
DISCLAIMER
| Having been informed that the organization of Fleetwood Fastpitch is to provide supervised Fastpitch games for the children, I/we, the parent(s)/guardian(s) of the named child hereby give our approval to his participation in any and all of the activities during the current season. We assume all the risks and hazards incidental to the conduct of the activities, transportation to and from the activities and We do further hereby release, indemnify and hold harmless the Fleetwood Fastpitch, the organizers, managers, umpires, sponsors, supervisors, and any other such person and/or all of them. In the case of injury to our child, We hereby waive all claims against the above mentioned persons. We likewise release from responsibility any person(s) transporting our child to or from the activities. We are in a position to furnish upon request, a certified copy of the birth certificate of the named child. We will guarantee the return of the uniform and equipment loaned to this child, returning it to the coach immediately following the season's end.
